Forms for Product VOC Emission Testing

When you submit a product sample for VOC emission testing at our lab, the sample must be accompanied by a completed and signed chain-of-custody (COC) form that includes:

• Client and manufacturer information
• Product sample name and identification
• Manufacturing, collection, and shipping dates for the product sample
• Requested chamber test protocol (including sampling times)
• Intended application of test data (CHPS, FloorScore, etc.)
